You can request prescription refills in several convenient ways:

Contact your pharmacy and request an electronic refill

Call our office at 847-741-0026.

Submit a refill request through our secure online patient portal.

To avoid delays, please request medication refills at least 3–5 business days before you run out.

Your co-pay and any outstanding balance are due at the time of your appointment. Please bring your current insurance card to each visit so we can verify coverage.

If you are unable to pay your balance in full, our office is happy to discuss flexible payment plan options. Visit our Billing Questions page for more details about insurance and payment policies.

Please arrive 15 minutes before your scheduled appointment time to allow for check-in and paperwork. 

For a smoother visit, please bring:

• A valid photo ID
• Your insurance card
• A list of current medications
• Any relevant medical records or referral forms

We strive to see patients in a timely and efficient manner. Most patients are seen within one hour of their scheduled appointment.
