When specialized evaluation or treatment is needed, our team helps connect patients with experienced specialists and imaging centers throughout the Elgin and Fox Valley area.
We work closely with a network of specialists to support timely referrals, preventive screenings, chronic disease management, and continuity of care. Our goal is to make the referral process as smooth and convenient as possible for our patients.

- Gastroenterology: Including screening colonoscopies and digestive health evaluations
- Mammography & Imaging Centers: Breast imaging and preventive screening services. Note: Many mammogram appointments do not require referrals.
